Indoren (Indomethacin) is a non-steroidal anti-inflammatory agent (NSAID) that inhibits cyclo­oxygenase, which is necessary for the formation of prostaglandins and other autacoids. It also inhibits the motility of polymorph-nuclear leukocytes.

Indications
Indomethacin is used to treat rheumatoid arthritis ankylosing spondylitis, osteoarthritis, acute painful shoulder (bursitis and /or tendonitis), and acute gouty arthritis.

Mechanism of action
Indomethacin is a cyclo­oxygenase or COX inhibitor that acts on both COX-1 and -2 to inhibit the synthesis of prostaglandins. COX synthase catalyses the conversion of arachidonic acid to a number of prostaglandins involved in fever, pain, swelling, inflammation, and platelet aggregation.
Side effects
Gastrointestinal disturbances, headache, dizziness, also gastrointestinal ulceration and bleeding, rarely drowsiness, confusion, insomnia, convulsions, psychiatric disturbances, depression, syncope, blood disorders (particular thrombocytopenia), hypertension, hyperglycemia, blurred vision, corneal deposits, peripheral neuropathy, and intestinal strictures.
Contra indications
Indomethacin is contraindicated in patients with known hypersensitivity to indomethacin.
Indomethacin should not be given to the patients who have experienced asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs.
Indomethacin is contraindicated for the treatment of peri-operative pain in the setting of coronary artery bypass graft (CABG) surgery.
Drug interactions
Severe, sometimes fatal, toxicity has occurred following administration of a NSAIA (e.g., indomethacin, ketoprofen) concomitantly with methotrexate (principally high-dose therapy) in patients with various malignant neoplasms or rheumatoid arthritis.
